How we make money: one company on this page — Coreage Rx — pays us a commission when a reader signs up through our link. It buys them nothing else: not a better score, not a higher place in the table, not the removal of a single criticism. Our scoring method and full disclosure are both public.

Zepbound is Eli Lilly’s brand of tirzepatide, approved for weight management. Self-pay pricing has been listed around $299 to $499 a month, which is what sends most people looking for alternatives.

The realistic alternatives fall into two groups: compounded tirzepatide, which is the same molecule prepared by a pharmacy and not FDA-approved, and a different drug entirely such as semaglutide or one of the new approved pills. This page covers the first group, priced honestly.

What gets a programme onto this table

  • Delivers tirzepatide — the same active ingredient as Zepbound.
  • Monthly price includes the medication.
  • Sorted cheapest first on the billed figure.

18 of the 21 providers we track qualify. Every figure checked by hand and verified 2026-09-18 and re-checked periodically.

The list

Every row below is priced against Zepbound at $499 a month, and sorted by how much it saves you over a year. We use the $499 self-pay figure listed for the 5 mg, 7.5 mg and 10 mg single-dose vials; lower starting doses have been listed nearer $299. Ranked on our overall score rather than price alone — the cheapest option is on the cheapest tirzepatide page.

Questions we get asked

Is compounded tirzepatide the same as Zepbound?

Same active ingredient, different product. Zepbound is manufactured and FDA-approved; compounded tirzepatide is prepared by a pharmacy against a prescription and has not been reviewed by the FDA for safety, effectiveness or manufacturing quality.

Is there a cheaper way to get Zepbound itself?

Possibly. Manufacturer self-pay channels have brought the price down considerably, and from July 2026 eligible Medicare beneficiaries may access certain GLP-1s for $50 a month.

Two things worth knowing before you buy on price

Compounded preparations are not FDA-approved. Two approved oral GLP-1s now start around $149 a month, and from 1 July 2026 eligible Medicare beneficiaries can access certain GLP-1s for $50 a month — both of which beat a large part of this market. See the approved pills and the Medicare programme.

In SURMOUNT-1, adults with obesity lost an average of 19.5% of body weight on 10 mg and 20.9% on 15 mg over 72 weeks. That figure comes from a controlled trial of the active ingredient, not from any particular seller's product.
