Medicaid no longer covers GLP-1s for obesity here. What that means, the indications that are still mandatory, and what it costs if you pay cash.

Coverage beats every cash price on this site. In Pennsylvania the question is whether you can get it — and the answer moved in the last year.
Pennsylvania eliminated Medicaid coverage of GLP-1s for obesity treatment.
Pennsylvania dropped coverage of GLP-1s for obesity after October 2025. KFF also lists Pennsylvania among the states that cover one or more weight-loss drugs but do not cover any GLP-1 specifically — so a weight-loss benefit existing on paper does not mean a GLP-1 is available under it. Everything below is sourced — we are not going to tell you what a Pennsylvania clinic charges, because every provider we track is 50-state telehealth and prices the same wherever you live. What actually differs by state is coverage, and that is what this page is about.
What it is worth in money
Medicaid enrollees generally pay little or nothing for covered prescriptions. Against our cheapest verified cash programme at $86 a month, coverage is worth around $1,000 a year — more than any discount, membership trick or annual plan on this entire site.
Why Pennsylvania is different
Pennsylvania appears twice in KFF's analysis, and the two entries pull in different directions: it is listed among the four states that eliminated GLP-1 coverage for obesity after October 2025, and separately among the seven that cover one or more weight-loss drugs while covering no GLP-1. Read together they describe a state where a weight-loss benefit exists on paper and a GLP-1 is not available under it.
If you are on Medicaid in Pennsylvania
What to do in Pennsylvania: Ask specifically whether semaglutide or tirzepatide is on the formulary rather than asking whether weight-loss medication is covered. In Pennsylvania those questions have different answers.
Coverage for type 2 diabetes, cardiovascular disease and sleep apnea is mandatory everywhere, so a refusal framed around weight loss is not the end of it — and on Medicare, the $50 programme starts 1 July 2026 regardless of which state you are in.
Paying cash in Pennsylvania
Providers are licensed across all 50 states and charge the same here as anywhere, so there is no local price to hunt for — the cheapest verified all-in rates are SuperHealthy Rx at $85 for semaglutide and Trimi at $125 for tirzepatide. Full tables in the cheapest tirzepatide. If you buy compounded, check the pharmacy licence with the Pennsylvania State Board of Pharmacy,
Pennsylvania: common questions
Pennsylvania says it covers weight-loss drugs. Why can I not get semaglutide?
Because those are two different statements. KFF lists Pennsylvania both as having eliminated GLP-1 coverage for obesity and as covering one or more weight-loss drugs while covering no GLP-1. The benefit exists; the GLP-1 is not on it.
Is there any route left through Pennsylvania Medicaid?
The mandatory indications — type 2 diabetes, cardiovascular disease, obstructive sleep apnea with obesity. Those are not optional for any state, including this one.
