Louisiana is one of the seven states KFF identifies as covering at least one weight-loss drug while covering no GLP-1. As of April 2026, however, state lawmakers were debating whether to allow Medicaid to cover GLP-1s for obesity where an enrollee has another chronic condition — prediabetes, hypertension or cardiovascular disease. That would be a narrower route than full coverage, but a route. Why Louisiana is different
Louisiana combines a weight-loss benefit that excludes GLP-1s with some of the highest obesity prevalence in the country — it sits in the South alongside Mississippi and West Virginia, the two states above 40%. The result is a state where the medication is most needed and least reachable through the public programme.

What to do in Louisiana: Ask about the diabetes and cardiovascular indications specifically. In a high-prevalence state a larger share of people will qualify under one of the mandatory routes than assume they do.
Coverage for type 2 diabetes, cardiovascular disease and sleep apnea is mandatory everywhere, so a refusal framed around weight loss is not the end of it — and on Medicare, the $50 programme starts 1 July 2026 regardless of which state you are in.

The two are not connected in the way you would hope. Coverage of GLP-1s for obesity is optional for every state regardless of prevalence, and KFF reports cost as the dominant factor in these decisions rather than clinical need.
What is the best route in Louisiana?
The mandatory indications. In a state with high obesity prevalence, a larger share of adults also carry a qualifying diagnosis such as type 2 diabetes than generally assume they do.
